Ascension All Saints Hospital and the Racine Fire Department have developed a program to assist patients recently discharged from the hospital. The Mobile Integrated Health program will begin this month and it will allow paramedics to visit patients at their homes to conduct safety inspections, as well as make necessary recommendations to prevent further illness. During their patient visit, paramedics will discuss long-term care. including diet and exercise, among other concerns.
This program aims to provide patients a comprehensive plan for recovery and help avoid follow up visits to the hospital within a month of being discharged. That could cut costs for both the fire department and hospital.
For the program’s trial period, it will focus on patients with serious conditions such as congestive heart failure, obstructive pulmonary disorders and cardiovascular strokes.
